2. Kindly keep your photograph, proof of age, degrees and certificates ready.
3. You can either PAY ONLINE or OFFLINE
4. Upon saving of the form, a username and password will be generated. Kindly copy and keep it safely as it will be
required to complete the form submission process.
5. For offline mode, keep your Payment Receipt/Cheque No./UTR No./Transaction ID handly.
6. Please be aware that, for security reasons, the form session is active only for a limited time. If the browser
window remains inactive for more than 30 minutes, the form submission will not be processed, and you will need to
restart the submission process.
